What is a PCN?
Formed in 2019 Primary Care Networks (PCNs) are the newest solution to collaborative working within the NHS with the aim of sharing resources to tackle health demands of the area. Individual GP surgeries have come together with community, mental health, social care, pharmacy, hospital, and voluntary services to produce proactive, personalised, coordinated and more integrated health and social care for people close to home.
